The Anna T. Jeanes Foundation, which supports Jeanes Hospital, is accepting applications for its 2014 community grants program, which is designed to provide funding for projects that promote the health, wellness and safety of communities in the hospital’s service areas.
Grants are awarded annually based on merit, community impact and fund availability. This year’s grants will be awarded up to $2,500.
“We are actively seeking ways to work in concert with other organizations to promote healthy lifestyles, safety for our neighborhoods and access to services for those who are underserved. This grants program funds these opportunities,” says Linda J. Grass, president and CEO of Jeanes Hospital.
The deadline for grant applications is Monday, Nov. 24. All applications must meet the following eligibility criteria:
• Applicants must be nonprofit organizations that are able to provide a copy of their tax exemption letter from the Internal Revenue Service and a completed W9 form.
• All applications must be submitted on the official application form. You may use additional paper if necessary, or enclose additional materials. Call 215–728–2131 for an application form or email [email protected].
• Organizations applying for grants must be located in one of the following ZIP codes: 19012, 19027, 19111, 19115, 19120, 19149, 19152, 19006, 19046, 19095, 19114, 19116, 19124, 19126, 19135, 19136 and 19154. Projects outlined in the grant applications must take place within these ZIP codes.
• Funds awarded must be used to promote health, wellness and/or the safety of community residents. Funds must be used between Jan. 1, 2015 and Dec. 31, 2015.
